Welcome to MY SCHD#1 Patient Portal

My SCHD#1 Patient Portal gives you direct online access to portions of your electronic medical record where your health care provider stores your health information. Your diagnostic results, prescription medicines, allergy information, and more, are all securely stored in this electronic record for quick retrieval. My SCHD#1 shows you that information-so you see what your provider sees!

My SCHD#1 also provides a centralized electronic method to view all pending appointments scheduled through our Specialty Physician Clinic.


Why is Open access important to Patients?

Open access to your medical record is a way for you to become more informed and involved in your care SCHD#1 invites you to review your healthcare records through our convenient patient portal.

Having the chance to review and discuss specific health records with your physician or family member can help you take better control of your health and health care. Patients with access to their own records feel more in control of their care and gain a better understanding of their health. Chances are that you will have a better recall of your care plan and you are also more likely to take your medications as prescribed.


How to enroll in the my SCHD#1 patient portal

The next time you receive services at Sumner County Hospital District #1, you will receive enrollment information describing MySCHD#1 access. At that time, you will have an opportunity to fill out the Patient Portal access Enrollment form. Once filled out and turned in, the Health Information Management department will process the form, and send a TEMPORARY username and password to the email address provided on the Enrollment Form.  You must log on to the portal within 72 hours of receiving the TEMPORARY username and password. At the end of 72 hours the temporary username and password become  invalid and you will need to contact SCHD#1 Medical Records to request a new one.

You must be 18 years or older to enroll in the MY SCHD#1 Patient Portal.
