We Need Your Help!

By answering these assessment questions, you will help us identify health needs in our community and service area. The ANONYMOUS results will assist us in implementing a community-wide plan for a healthy future by guiding additional services to include in a new facility and identifying areas for improvement to the current services we provide.

• Who do we NEED to participate?
o All Residents (18years & Older) of our County Tax District

• Who would we LIKE to participate?
o All Residents (18years & Older) of Sumner and Surrounding Counties (Cowley & Harper, KS Grant & Kay, OK)
o Who have or would like to utilize the services provided by the hospital and/or clinic in Caldwell, KS
